Prenatal Care Coordination (PNCC)

An assigned nurse visits you at your home or other preferred location throughout your pregnancy to provide you with a variety of educational materials. An individualized plan is made after the first visit to meet the needs of the client and focus on areas of concern. This personalized plan helps keep the mother and baby healthy and focused through the end of their pregnancy.

  • Breastfeeding
  • Car seats
  • Childbirth
  • Cribs
  • Common discomforts of pregnancy
  • Depression risk
  • Warning signs of pregnancy issues
 
  • Fetal Growth and Development
  • Home safety
  • Maternal health and wellbeing
  • Nutrition
  • Signs of early labor
  • Stress management
Other services include:    
  • In home lead, water, and radon
    testing
  • Blood pressure checks
  • Helping to quit smoking
 
  • Referrals to reproductive health for post pregnancy birth control methods

Post Partum Newborn Care (PPNB)

After the baby is born the hospital will ask if you are interested in having a nurse come to your home to do a visit to check in on the mother and baby. If you indicate that you would like a visit the hospital will then send a referral to the health department. This service is available to clients seen during the prenatal period and also to all other mothers.

  • Listen to heart and lung sounds
